The Customer Feedback Survey starter workbook: a Survey_ sheet with five sections of rated, choice and open questions, a department list and a results dashboard. Enter your email to download it and join the early-access list. Enter your email to download the workbook and join the early-access list.

Joining the waitlist means we store your details and send you the messages needed to run it — your place in the queue, your invitation and launch access details. Those are service messages you ask for by joining; marketing email only with the box ticked.

Survey tools make you rebuild your questions in someone else’s editor, then trap the responses there. In Sheetward the survey is a sheet: one question per row — its section, wording, response type and choices — that generates the survey page, the response records and the results dashboard together.

How it works

1

Open the template

Reword the questions, adjust the rating scales and choice lists, add or remove sections.

2

Upload & review

Sheetward lists every section, question and response type it detected — including the automatic “Other (please specify)” follow-up. Confirm, and the app is live.

3

Publish & share

Publish the app and share the link or QR code. Respondents answer on any device — no account, no app install — and results appear on the dashboard as they arrive.

What the template becomes

Questions as rows

A Survey_ sheet holds the whole questionnaire — Section, Question, Response, Choices, Required — so editing the survey is editing a spreadsheet.

Every response type

Star ratings on any scale, single and multiple choice, short text and long comments — plus real data types like numbers, currency and phone where a question needs one.

“Other (please specify)” built in

Offer Other (please specify) as a choice and Sheetward adds the follow-up text field automatically — revealed only when Other is chosen, and required exactly then.

Automatic numbering

Sections and questions number themselves at display time — continuous or per-section — so inserting a question never means renumbering the sheet.

A shareable survey page

Publishing produces a clean one-column survey — sections as cards, big rating stars, radio and checkbox groups — that respondents fill without signing in.

Results, not just rows

A Report_ sheet becomes the results dashboard: average ratings, NPS-style scores and answer distributions, live as responses arrive — and every response exports to Excel or CSV.

Why teams pick Sheetward

  • The survey, the data and the results live in one app you generated — not three tools stitched together.
  • Respondents need no account: one link or QR code, any device.
  • Flat pricing — surveys and forms share the same allowance, no per-response fees.
  • Own your data: export every response anytime, or run the app as a standalone bundle.

Frequently asked questions

Do respondents need a Sheetward account?

No — a published survey is a public, intake-only page: respondents see the survey and nothing else. Anyone with the link can respond; you control publishing, can rotate the link, and can close the survey when the collection window ends.

Can I mix a survey with regular forms in one app?

No — a survey app is its own kind of app, so Survey_ and Form_ sheets live in separate workbooks (several surveys can share one). For plan limits, surveys and forms still count toward the same allowance.

How do I see the results?

The template ships a results dashboard (a Report_ sheet) with rating averages and answer counts, and every response is a record you can filter, chart and export.

Is the template required?

No — any workbook with a Survey_ sheet works; the template saves you writing the sections, scales, choices and dashboard from scratch.
