A Survey_ sheet holds the whole questionnaire — Section, Question, Response, Choices, Required — so editing the survey is editing a spreadsheet.
Every response type
Star ratings on any scale, single and multiple choice, short text and long comments — plus real data types like numbers, currency and phone where a question needs one.
“Other (please specify)” built in
Offer Other (please specify) as a choice and Sheetward adds the follow-up text field automatically — revealed only when Other is chosen, and required exactly then.
Automatic numbering
Sections and questions number themselves at display time — continuous or per-section — so inserting a question never means renumbering the sheet.

Publishing produces a clean one-column survey — sections as cards, big rating stars, radio and checkbox groups — that respondents fill without signing in.

Frequently asked questions
Do respondents need a Sheetward account?
No — a published survey is a public, intake-only page: respondents see the survey and nothing else. Anyone with the link can respond; you control publishing, can rotate the link, and can close the survey when the collection window ends.
Can I mix a survey with regular forms in one app?
No — a survey app is its own kind of app, so Survey_ and Form_ sheets live in separate workbooks (several surveys can share one). For plan limits, surveys and forms still count toward the same allowance.
How do I see the results?
The template ships a results dashboard (a Report_ sheet) with rating averages and answer counts, and every response is a record you can filter, chart and export.
Is the template required?
No — any workbook with a Survey_ sheet works; the template saves you writing the sections, scales, choices and dashboard from scratch.
